Sadly, I cannot help with most of your questions. As for other episodes, the only one I can think of at the moment is the s6 SG-1 episode "Redemption." McKay tells Sam about how when he was growing up he wanted to play the piano, and about how his parents hated eachother & blamed him. I don't believe he mentioned where he grew up, through. So, I'm not sure how much help that episode would actually be...
